How to I take part?

Why have you asked me?

A GP or nurse from your GP practice thinks that you:

have more than one long-term health condition
are between 18 and 65 years old.

What happens if I decide to take part?

1. You fill in a survey to tell us more about:

the effort you have to put in to looking after your long-term health conditions
your experience with healthcare services.

The survey should take about 30 minutes.

2. If you agree, we will collect some information from your healthcare records:

age and sex
how many healthcare appointments you have at your GP practice
medications you take
number and type of health conditions you have
how wealthy the area you live in is.

If you wish, you can complete the survey but not give us permission to collect information from your healthcare record.  We will keep your information safe and will NOT share your answers with your GP practice. We will prepare reports but no one will be able to identify you from the information. Your answers will not affect your healthcare.
